ABSTRACT

Objective To investigate the changes of Caprini score, plasma D-D and FIB levels in patients with post-fracture DVT and its diagnostic significance. Methods Sixty patients with post-fracture DVT (DVT group) and 120 patients without DVT after fracture were selected (control group). The Caprini score, plasma D-D and FIB levels were compared between the two groups. The relationship between Caprini score and plasma D-D and FIB in the DVT group was analyzed. Results The Caprini score, D-D and FIB levels in the DVT group[7.26±1.88, (3.31±1.52) mg/L, (4.48±0.72) g/L] were significantly higher than those in the control group[3.39±0.80, (0.46±0.11) mg/L, (3.60±0.68) g/L], and the difference was statistically significant (P<0.05).There was a significant positive correlation between Caprini score and D-D and FIB levels in patients with DVT (P<0.05).There was a positive correlation between Caprini score and level of D-D and FIB (r=0.748、r=0.662, P<0.05).The sensitivity of Caprini score combined with D-D and FIB levels in the diagnosis of DVT in fracture patients was 97.83%, specificity was 92.00%, missed diagnosis rate was 2.17%, misdiagnosis rate was 8.00, and area under ROC curve was 0.947 (P<0.05).Conclusion Caprini score, D-D and FIB levels in patients with lower limb fracture have certain clinical value for the diagnosis of DVT.

ABSTRACT

Objective To investigate the changes of fibrinogen and D -dimer levels in patients with severe burn and their relationship with prognosis .Methods 90 patients with severe burn were selected as the subjects .The patients were divided into the dead group (30 cases) and the survival group (60 cases) according to the prognosis of the patients. The differences and changes of fibrinogen and D -dimer at the same time after admission were compared between the two groups .Results At each time point after admission , the fibrinogen levels of the death group were significantly higher than those of the survival group at the same time [(5.9 ±1.5)g/L vs.(5.1 ±1.3)g/L,(8.8 ± 2.2)g/L vs.(7.4 ±2.5)g/L,(7.6 ±2.8)g/L vs.(5.7 ±1.2)g/L,(7.1 ±2.5)g/L vs.(4.3 ±1.7)g/L],the differences were statistically significant(t =3.892,4.762,5.436,6.987,all P <0.05).At each time point after admission, the D-dimer levels of the death group were significantly higher than those of the survival group at the same time[(743.4 ±226.2)ng/mL vs.(704.1 ±214.6)ng/mL,(935.4 ±256.9)ng/mL vs.(834.6 ±256.2)ng/mL, (846.3 ±291.5)ng/mL vs.(712.1 ±251.3) ng/mL,(756.7 ±232.8) ng/mL vs.(601.5 ±168.7) ng/mL],the differences were statistically significant (t=3.895,5.477,4.743,5.746,all P<0.05).The level of D-dimer in the survival group showed a rise firstly ,then showed a decrease trend after admission ,while the levels of fibrinogen and D-dimer of the death group were altered in double -peak change .Conclusion There is an obvious impairment of coagulation function in severe burn patients , and the level of D -dimer and fibrinogen is higher , the prognosis of patients is worse .

ABSTRACT

Objective To study the influencing factors of raised D-dimer levels in elderly people. Methods 592 elderly people underwent physical examination in our hospital were selected. They were divided into observation group and control group according to the level of D-dimer.The clinical data, liver and kidney function and blood lipid levels between both groups were compared.Re-sults There were no significant difference on gender,BMI,FBG,ALT,AST and Cr between both groups (P >0.05),but the age of patients in the observation group were significantly higher than that of the control group(P <0.05).The PT,CRP,bacterial pneu-monia and (or)of acute exacerbations of chronic bronchitis, cancer, diabetes patients in the observation group were significantly higher than those in control group, while LDL-C,TC were significantly lower than that of the control group, there were significant differences (P <0.05).The plasma D-dimer level in patients showed positive correlation with CRP and age (r =0.27,0.25,P <0.05).Conclusion Malignant tumor, type 2 diabetes, bacterial pneumonia, acute exacerbation of chronic bronchitis are the risk fac-tors of raised plasma D-dimer levels in the elderly.

ABSTRACT

Objective To study the meaning of Homocysteine, serum high sensitive C-reactive protein, lipoprotein A and D-two dimer combined detection in the course of coronary heart disease. Methods Select 180 cases suffering from different types of coronary heart disease as group of coronary heart disease,200 healthy people who were in our hospital physical examination qualified over the same period as the control group, all the cases of fasting venous blood was sampled respectively to detect the content of Hcy,hs-CRP,LP(a) and DD.Results Coronary heart disease group of patients with Hcy,hs-CRP,LP(a) and DD levels were significantly higher than those of the control group of healthy persons(t =13.87,10.24,9.86,18.02,P < 0.05),Comparison among different indicators of the patiens with Coronary heart disease:the group of AMI was higher than the group of UAP (t =15.79,9.73,11.47,17.34,all P < 0.05),the group of UAP was higher than the group of SAP (t =16.82,8.14,10.65,18.27,P <0.05),the group of SAP was higher than the group of SMI (t =14.36,10.35,9.98,16.52,P < 0.05).Conclusion Monitoring of Hcy,hs-CRP,LP(a) and DD for coronary heart disease, contributes to the understand of the development of the course of coronary heart disease and assess the risk of coronary heart disease.

ABSTRACT

Objective To investigate the D-in patients with severe trauma in two dimers (D-D),fibrinogen (Fbg) and platelet parameters (PLT,MPV,PDW) level induced by postoperative deep vein thrombosis (DVT) in the formation of value in clinical diagnosis. Methods Eighty cases of healthy people as control group, were compared before and after operation and level and 321 severe traumatic patients were measured before surgery, after 1 days and 3 days of D-D,Fbg and platelet parameters, trauma were divided into fracture, traumatic brain injury, multiple injuries in three groups. Results Different trauma patients with D-D,Fbg concentration and platelet index compared with the control group increased obviously significant (P < 0.05),especially the brain trauma and multiple injuries in two group increased significantly (P <0.01),the positive rate of D-D was 22.8%,35.2%,40.9%.Postoperative 1D in addition to brain injury group, trauma group D-D levels compared with those before operation were significantly increased with significant (P < 0.05),other indicators compared with preoperative changes little, the positive rate of D-D increased slightly, but without statistical significance.Levels of Fbg 3D after the trauma group decreased significantly,D-D increased significantly, decreased the number of PLT,MPV,PDW value increases, the value of preoperative has the remarkable significance (P < 0.01 or P < 0.05).The positive rate of D-D was increased up to 41%,59.2%,62.3%,compared with those before operation by chi-square test was significant (P <0.01).Conclusion The patients with severe trauma patients after two D-dimer concentration should be alert to the occurrence of DVT,contribute to the clinically as soon as possible to take measures to avoid, there is an important clinical significance in the treatment and prognosis of trauma patients.
